Athletic Requirements and Forms

Pre-participation Paperwork
In order to participate in the athletic period, any practice and/or performance including tryouts, all student athletes are required to annually complete the entire Galveston ISD participation packet which includes the following:  Student/Parent/Guardian information, Informed Consent, UIL Acknowledgement of Rules, UIL Steroid Agreement Form, Assumptions of Risks, Medication Permit, Helmet Safety (football), Permit to Travel, Athletic Insurance Disclaimer, UIL Medical History Form, and UIL Physical Form.  These forms must be cleared and on file in the Athletic Training office prior to participation.  Other forms to participate come from each head coach and must be filled out and signed prior to participation.  These include team rules, athletic codes of conduct, and all expectations.  Participation in athletics is a privilege and not a right and participation may be revoked if forms are incomplete, team rules are broken, or there are disciplinary issues that have been violated.

Academic Eligibility
All academic eligibility requirements for extracurricular participants are outlined the Texas Education Agency/UIL Side-by-Side document - 2010-2011 T.E.A./U.I.L. Side-by-Side
 
At the high school level the following academic requirements apply for the beginning of the school year:
  • Incoming 9th graders must have been promoted and not placed
  • 10th graders must have passed 5 state credits from the prior year
  • 11th graders must have passed 10 state credits total or 5 state credits from the prior year
  • 12th graders must have passed 15 state creidts total or 5 state credits from the prior year
At the middle school level the following academic requirements apply for the beginning of the school year:
  • Incoming 7th graders must have been promoted and not placed
  • 8th graders must have been promoted and not placed
Any student that does not meet the criteria above is ineligible to participate in games and/or contests the first six weeks of the school year or until they pass all classes at the first official grade reporting period.  Students may practice however.  The first official grade reporting period is the first six weeks report and the second official grade reproting period is the first nine weeks report.
 
The remainder of the school year follows the Galveston ISD Eligibility Calendar.  The high school and middle schools are on a nine weeks grading system.
