Required Consent Forms

Due to new legislation for the 2025–26 school year, parents/guardians are required to opt in or opt out of certain services that were previously provided to students.

What you need to complete:

• Health Services consent — one per student (Grades K–12, ECSE, and ATS)
• Counseling Services consent — one per student (Grades K–12, ECSE, and ATS)
• Club/Extracurricular Participation consent —one for every club/organizations

Please complete the required forms for each student enrolled. If you have multiple students, you’ll need to submit separate forms for each child.
