Required for students enrolling in a Texas public school for the first time

Home Language Survey must be submitted.

The state of Texas requires a Home Language Survey be completed for each student who enrolls in a Texas public school for the first time. It is the responsibility of the parent or guardian — not the school — to provide the language information requested on the form.

This form can be found on this page below, or at your student's campus. The form must be submitted on or before the student's first day of school if they are entering a Texas public school for the first time. If you have previously completed a Home Language Survey, it must be provided to GISD and kept as part of the student's permanent record.

How to submit

  • Download the PDF for ALL LANGUAGES and print only the page range for your language (see the Table of Contents).
  • Return the completed form to your student’s campus office — the campus registrar can upload it to Skyward if required for the permanent record.
  • If you need help, contact your campus registrar.
