Dietary Accommodations

DOES YOUR CHILD HAVE A FOOD ALLERGY OR A DIETARY ACCOMMODATION REQUIREMENT?

Students with food allergies or dietary accommodation requirements may have an alert posted in the cafeteria point of sale. In order to do that, the parent/guardian must have a medical authority to complete the GISD Diet Modification Form. This form is to be filled out properly and signed by the parent/guardian and by the physician, and returned to the Front Office/School Nurse.

Please be aware that turning in this form will restrict all foods containing the allergens and/or special accommodations listed when the student eats in the school cafeteria.

This form is not required/necessary for students who have mild food allergies or intolerances, unless those ingredients need to be completely restricted from the child's diet.

Incomplete forms will result in delays to dietary accommodations being processed.

If you have any questions on dietary accommodations, food restrictions or the Diet modification form, please feel free to email our Nutrition Supervisor Mrs. Arlene Saldana at arlenesaldana@gisd.org or at 409-766-5162.
